Brain Stem Stroke

The brainstem is part of the brain and is located at the bottom of the skull, in line with the spinal cord. It connects the cerebrum to the cerebellum and spinal cord. The brainstem consists of three parts: the medulla, pons and midbrain. The brain stem is responsible for basic functions such as our body temperature, heart rate, breathing, blood pressure, digestion, eye movements, urination, hearing, taste, chewing and swallowing, and feeling movement and gravity.

A brainstem stroke can be both an infarction and hemorrhage in the brainstem. In an infarction, there is a blockage of a blood vessel by a blood clot, also known as thrombosis or embolism, or by a narrowing of a blood vessel. Bleeding occurs suddenly due to a tear in a blood vessel or the bursting of a blood vessel, for example due to an aneurysm. Both an infarction and a hemorrhage cause a disturbed blood supply to the brainstem. As a result, oxygen-rich blood can no longer reach this brain area properly, causing an oxygen deficiency and the brain stem can no longer function properly. Brain tissue without blood flow quickly dies. Early treatment is therefore essential to limit the damage.

Symptoms of a brainstem stroke may include dizziness, vertigo, balance problems, double vision, swallowing difficulties, slurred speech, and impaired consciousness.

The risk factors for stroke in the brainstem are the same as for strokes in other parts of the brain, including high blood pressure, diabetes, heart disease, atrial fibrillation, and smoking.

The consequences of a brainstem stroke depend on the severity of the stroke. There may be loss of strength, hoarseness, sweating, eye twitching, balance problems and problems with proprioception.
